Enrollment Forms and Instructions: Infant & Toddler Program

The first step in the enrollment process is to submit an Infant and Toddler Interest Form and a $30 processing fee. In addition to this form, you will need to download and complete the forms listed below. Please return completed forms to The Village office as soon as possible. When all of your enrollment forms have been received, The Village will be able to determine placement for your child. 

Completed enrollment files must contain:

  1. Completed enrollment forms:
    Emergency Card
    Family Intake Form

    Release Form
     
  2. Copy of child’s official birth certificate
  3. Child’s immunization record
  4. Proof of address: house closing papers or rental/lease agreement
  5. Health History Form
  6. Nutrition History Form

 A completed Health Status form is due at the start of school. This form needs to be completed by qualified medical staff and will be reviewed by our school nurse. Health Status forms expire one year after the date of the exam.

Please include any comments/concerns or other information you would like us to know about your child on a separate sheet of paper and submit it with your packet.
